The South East Local Health Integration Network (LHIN) is one of 14 networks established by the province to plan, manage, and fund the health care system at the local and regional level. Promoting integration and networking among our health care resources is already showing results. The South East region extends from Brighton on the west to Prescott and Cardinal on the east, north to Perth and Smith Falls, and back to Bancroft.


Position:  Manager, Home and Community Care - Location Brockville

Region:  Leeds & Grenville

Description:  

As the Manager of Home and Community care, you will be responsible for ensuring appropriate assessment of needs and coordination/management/provision of services. The position manages day-to-day operations to ensure that effective and efficient services are provided to patients and their families through the provision of case management and long-term care home placement services.
More specifically, in this role you will:

Patient Service Delivery
  • Manages the delivery and/or access to coordinated, quality community health care;
  • Acts as the primary contact for patient/family complaints or MPP concerns on issues not resolved by Care Coordinator or team member;
  • Participates, supports and/or leads in planning, implementing, managing, and evaluating patient service delivery programs;
  • Monitors and reviews caseload statistics and indicators for assigned portfolio and team;
  • Oversee the utilization of purchased patient services for the organization.

Relationship Management
  • Builds and maintains effective relationships with health and social service agencies and external service providers to partner in delivering quality patient care and promote an integrated approach;
  • Represents the South East LHIN on multi-disciplinary committees and community agency/project working groups.

Management of Human Resources and Leadership
  • Manages overall performance of local care community team; provides direction on projects and priorities; orients and trains staff to equip them with the necessary skills needed to resolve issues and provide quality patient care; guides Care Coordinators in handling critical issues;
  • Identifies staffing requirements and recommends recruitment and selection of candidates ensure
  • effective staff allocation within assigned budget.
We seek a profile that includes:
  • University degree preferred in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work;
  • Holds a current unrestricted registration as a member of the associated College;
  • Five (5) years of progressively responsible experience in health care;
  • Management experience in a unionized environment with direct experience with change management preferred.
  • Experience and knowledge of service delivery in a health care environment;
  • Knowledge of direct care/case management models used in community health care organizations;
  • Good knowledge of community resources (e.g., services and programs), and roles of health care professionals;
  • Knowledge of challenges and issues, methods and techniques for outsourced/ contracted services and service providers;
  • Knowledge of tools, systems and databases used in patient service delivery and management.
  • Strong planning and organizing skills;
  • Effective communication skills to maintain a range of contacts with health professionals, service providers, regulatory colleges, educational institutions/Boards, and other organizations within the community;
  • Strong collaboration skills to participate on projects and committees with colleagues across the South East LHIN or local community agencies;
  • Ability to deliver information effectively and interpret data clearly in a variety of settings including one-on-one with staff and in team meetings;
  • Ability to deal constructively with a variety of contentious/difficult situations, discuss sensitive information and influence/persuade others to follow a recommended course of action;
  • Good coaching and communication skills to provide instructions and guidance to staff with respect to activities, challenges and questions.


Salary:  $85,381 to $99,884

Terms of Employment:  Temporary Full-time up to 2 years
